L-Methylfolate (5-methyltetrahydrofolate, or 5-MTHF) is the biologically active form of folate that the body can use directly, without any enzymatic conversion. This makes it essential for people carrying MTHFR gene variants (C677T or A1298C) whose enzyme is impaired, leaving them unable to activate standard folic acid efficiently. Because it enters the methylation cycle immediately, it directly supports the production of neurotransmitters (serotonin, dopamine, norepinephrine) and the reduction of homocysteine. L-Methylfolate is the recommended form for supporting mood, cognition, and pregnancy in those with or without MTHFR mutations. A small percentage of individuals experience over-methylation symptoms (anxiety, irritability) when starting — beginning at a low dose (e.g. 200 mcg) and increasing gradually usually resolves this.
